Form for arrival in Slovenia

A person may enter the Republic of Slovenia if he / she submits proof of vaccination against COVID-19, proving that the following has been received:
  - the second dose of Comirnaty by Biontech / Pfizer,
  - the second dose of Spikevax (COVID-19 Vaccine) by Moderna,
  - the second dose of Sputnik V has been produced by Russia‚Äôs Gamaleya National Center of Epidemiology and Microbiology,
  - the second dose of CoronaVac from Sinovac Biotech,
  - the second dose of Sinopharm COVID-19 Vaccine,
  - the second dose of Vaxzevria (COVID-19 Vaccine) from AstraZenec,
  - the second dose of Covishield by Serum Institute of India / AstraZeneca or a combination of the two vaccines listed above,
  - the first dose of COVID-19 Vaccine Janssen by Johnson and Johnson / Janssen-Cilag,

The part relating to Vaxzevria (COVID-19 Vaccine) from AstraZeneca and Covishield from Serum Institute of India / AstraZeneca will be used after 14 days from the date of entry into force (28 August 2021).

PASSENGERS FROM THE SAME HOUSEHOLD who are also users of the same exception *

* If there are people from different households in the same vehicle, everyone must have their own travel registration, the same applies if people from the same household benefit from different exceptions. An exception is also a child up to 15 years of age who crosses the border together with a close family member who is not sent to quarantine at home and has not been denied entry into Slovenia. The child is also added to the list.

At the border control use the same document as you specified in the form.
